1. You MUST have permission on Date of Death to create an individual provider. This is a known RAID 2759
2. You MUST have permission on Disciplinary Action Display flag to create a DA. I have added this to RAID 2767
3. A missing Status node is not handled correctly (Transaction 201 in the regression file). I haveraise new RAID 2856. THe only way around this for the moment is to comment the transaction out of the regression file.
